At Passion Care Homes, we offer a personalized care approach for 5 individuals in a residential group home setting. We believe individuals thrive best in an intimate home environment that they are familiar with and meets their unique needs and preferences, enabling them to contribute to their fullest potential. Meet Our Founder
Pa Yang, MSN, RN, PHN, LALD
Passion Care Homes was founded by Pa Yang who currently serves as the Licensed Assisted Living Director and Clinical Nurse Supervisor. Pa has her Bachelor of Science in Psychology and Master of Science in Nursing. With over 12 years of healthcare experience in cardiac nursing, care coordination and group homes, she created Passion Care Homes as an alternative to large long-term care facilities for individuals to receive personalized care. Growing up, seeing her grandparents placed into nursing homes and institutions, she wanted to provide options for families' loved ones to optimally "age in place" in a comfortable home-like setting. This is the vision and model of care in her residential group homes.
